Please click on the link above to find forms from Ohio Job and Family Services that are required for your child to attend preschool.  To find the necessary forms, follow under the heading for Providers and then click on the link Child Care Forms (pdf).  The forms you need for school are Child Enrollment/Health Information and Child's Medical Form.  You can print them out from the website.
